Meta-analyses that pool randomized controlled trials of egg consumption report a small but consistent rise in LDL cholesterol. A systematic review of 17 RCTs in healthy populations found the higher-egg group had elevated LDL cholesterol (mean difference roughly 8 mg/dL) and a higher LDL/HDL ratio, with little heterogeneity between trials; a separate pooled analysis reported an LDL-C increase of about 5.6 mg/dL, alongside smaller rises in total and HDL cholesterol. Increases on the order of a few milligrams per deciliter fit the claim's characterization of a "modest" effect, and no RCT meta-analysis reverses the pooled direction.

The finding rests on the direct causal result that adding eggs to a diet raises LDL relative to an egg-free diet, which is exactly what the pooled trials measure. Its magnitude and generality are qualified by the fact that egg-driven LDL increases are smaller, and sometimes absent, when eggs displace other foods rather than being added on top: because saturated fat is a stronger LDL driver than dietary cholesterol, isocaloric substitution designs and low-saturated-fat backgrounds attenuate the effect. This comparator-dependence limits how large and how universal the rise is, but it does not overturn the meta-analytic conclusion the claim reports.

Full reasoning: the evidence and decisions behind this verdict

The claim is about what the RCT meta-analytic literature reports, which is directly verifiable. Multiple independent meta-analyses converge in direction and magnitude: the Li Chen et al. synthesis (17 RCTs, healthy subjects; PubMed 32635569) reported LDL-C MD ≈ 8.14 mg/dL (p < 0.0001, I² = 18%) and LDL/HDL ratio MD = 0.14 (p = 0.001); Khalighi Sikaroudi et al. (PubMed 29111915) reported LDL-C +5.55 mg/dL (95% CI 3.14–7.69), TC +5.60 mg/dL, plus a smaller HDL rise; a 28-RCT meta-analysis (cited in the EVIDENT II paper, academic.oup.com/jcem) likewise found egg consumption increased TC, LDL-C and HDL-C; and the Nordic Nutrition Recommendations 2023 scoping review summarizes the same pattern. Effects of a few mg/dL match "modestly."
